Privacy Notice​

 

The information provided by people applying for assistance is obtained for the purpose of assessing the application. 

 

It will be used by the Leeton Jumpstart Fund and their appointed Assessment Committee to determine whether assistance can be granted and the level of support and financial assistance required. 

 

Provision of this information is voluntary.

 

The information will be stored securely and kept for 7 years for auditing purposes.

 

The information will only be disclosed for purposes directly related to the purpose for which it is collected.

 

If you do not provide all or any of this information then the Leeton JumpStart Fund and their Assessment Committee may not be able to assess your needs and the level of support and financial assistance that could be provided.

 

You may correct any personal information provided at any time by contacting the executive of the Leeton JumpStart Fund.

 

 

Client Confidentiality Agreement

 

I have read and noted the JumpStart Assessment Committee Privacy Notice.

 

I understand that during the process of assessing clients for financial assistance and support, the Leeton JumpStart Fund may have access to information, which is private and confidential.

 

I acknowledge that JumpStart and I will not convey to any person outside the Leeton JumpStart Fund any knowledge or information of a confidential nature which is gained in the course of this application.

 

Anonymity of the JumpStart Interview Panel is essential.

I understand the seriousness of any breach of this Agreement.
